    A Baby Step or A Leap?

    I think my colleague Joe gets the overview right .. “At their core, risk-based models should inspire the flexibility to deliver care in the manner most effective for patients’ overall health,”Joe Nicholson writes. But my experience over the years suggests that incrementalism and starting small leads to dilution, lack of influence, and confusion. I think that big steps are scary, but small steps can lead to activity without results.

    The challenge is to craft a plan that focuses on the patient at the center … and from that core transition out based on local conditions. Providers can only be successful if payers provide the environment that supports success for the providers they have influence with. In my mind that includes longitudinal information and insights – see Nascate, meaningful performance measurement tied to population health, and some version of subscription based payment e.g. professional capitation with TCC incentives.
